Before Your Appointment
Thank you for making an appointment to visit the offices of West Side Cardiology Associates. We ask that you bring the following with you when you come in for your visit:
- Patient Registration and Patient Medical History form: Please fill out the Patient Registration and Patient Medical History form as appropriate. It is easier to fill out these forms at home where you have all your records and someone can help you if you need it. You can download the forms by clicking on the links below:
- Insurance Card: Please bring your current health insurance card with you to check in more quickly.
- Medications: Please bring a list of your current medications, both prescribed and non-prescribed (over-the-counter), including vitamins, herbal supplements and anything else that you take orally on a regular basis.
